What to bring for your first office visit:

Below is a list of items which you should bring for your first appointment with Dr. Williams at Horizon Cardiology. The receptionist scheduling you for your first visit will go over most of these items, but you may also want to print this list for your own reference before your visit.

For new patients:

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)
  •  List of any medications that you may be taking and their dosages
    Copies of your medical history, x-rays, and/or lab reports if you have them
  • Any questions you may have for your doctor
  • We recommend you bring a friend or family member to your initial visit.
  • You can go online to fill out the forms.

For Follow up visit:

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)
  • List of any medications that you may be taking and their dosages
  • Any new tests results: x-rays, and/or lab reports
  • Any questions you may have for your doctor
  • Blood Pressure Monitor Chart
  • Blood Sugar Records

For the stress test:

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)
  • List of any medications that you may be taking and their dosages
  • Comfortable clothes and shoes for walking
  • Do not eat six hours before the stress test

For invasive out patient cardiac procedures:

  • Your insurance information and card (i.e., HMO, Medicare, Medicaid)
  • List of any medications that you may be taking and their dosages
  • Do not eat six hours before the procedure
